Weight regain in the postoperative period of bariatric surgery: sociodemographic, clinical, nutritional and genetic contributions

Abstract

The treatment of obesity aims at significant loss of weight, with consequent improvement of associated comorbidities and thus, several strategies can be used to induce a negative energy balance and consequent weight loss. Bariatric surgery has been a method of choice for cases of high degrees of obesity and has been shown to be an effective way to promote substantial weight loss. Long-term weight stabilization is one of the major challenges, as 20% of patients undergoing surgery regain weight after 5 to 10 years. The factors that lead to weight regain are complex, however they are not clearly established. In this context, the present proposal aims to identify the factors (nutritional and genetic) that determine weight regimens in the medium / late postoperative period (5 years) of bariatric surgery. This study will be evaluated in the period prior to the surgical procedure and during the postoperative follow-up (after 1, 2, 3, 4, 5 years): 1. Attendance at medical, nutritional and psychological appointments at the tertiary hospital; 2. Rate of complications of surgery and repercussion in the time of postoperative hospitalization; 3. Use of medications: oral hypoglycemic agents, lipid-lowering agents, antihypertensives, antidepressants, hypothyroidism, corticoids, anticoagulants, antianestritics and antidepressants; 4. Anthropometric parameters (weight, height, BMI and abdominal circumference) and from these, identify the weight regain; 5. Food consumption: energy value, intake of macro (carbohydrates, lipids, proteins) and micronutrients (iron, folic acid, vitamin B12); 6. Biochemical parameters and prevalence of chronic diseases (diabetes mellitus, dyslipidemias, obstructive sleep apnea syndrome, osteoporosis, cardiovascular diseases), as well as micronutrient deficiencies (iron, zinc and magnesium); 7. Genetic profile through microarray analysis (640,000 single nucleotide polymorphisms) in genes related to risk and predisposition to obesity. Finally, it will analyze which of the factors evaluated above influence or determine the weight regain in the medium / late postoperative period. (AU)
