A multicenter study for the clinical and anatomical evaluation of Acute Coronary Syndromes in patients with Cancer undergoing treatment in specialized centers (BRAzilian Registry of Acute Coronary Syndrome in Oncologic Patients - The BRAvAdO Registry)

Abstract

Procedure: Clinical and anatomical analysis of patients with Acute Coronary Syndrome (ACS) and diagnosis of Neoplasia. Population: Consecutively, all patients with a diagnosis of Neoplasia presenting with Acute Coronary Syndrome will be included. Objectives: To assess the prevalence of predisposing factors to the development of ACS in Cancer patients, (2) To evaluate the anatomical complexity of coronary lesions and total atherosclerotic load, through coronary angiography of patients presenting with Acute Coronary Syndromes , with and without diagnosis of Neoplasia. Secondary - (1) To propose the achievement of SCA risk score for oncologic patients, (2) To compare the incidence of major in-hospital cardiovascular outcomes (general death, cardiac death, nonfatal and new myocardial infarction revascularization).Design and study population: A multicenter, retrospective and prospective, observational study based on clinical data and coronary angiography. The angiographic analysis will consist of three analysis arms (two control groups) composed of: (1) patients with ACS and Cancer diagnosis, (2) patients with ACS and without diagnosis of cancer, and (3) patients who underwent coronary angiography in the context of ACS, to investigate CAD or to perform surgery (preoperative, except for surgery to treat Neoplasia). The initial plan is to include 200 patients in group 1, 200 patients in group 2 and 200 patients in group 3. Duration of study: 48 monthsAllocation: ConsecutiveIntervention/Follow-up: There is no intervention for included patients because it is an observational study. The study consists of the analysis of clinical data and coronary angiography of patients included in the study. Number of patients: 600 individuals. Centers involved: (1) Heart Institute (InCor) (main), São Paulo State Cancer Institute (ICESP), Cecap Clinical Cardiology Center and Sirio Libanês research Brasília, Hospital de Clínicas - Universidade Federal do Paraná, Centro de Teaching and Research "Prof. José Augusto Barreto", of the São Lucas Foundation (CEPeqFSL) - São Lucas Hospital, Institute of Cardiology - University Foundation of Cardiology, Paulistano-SP Hospital (Americas Oncology), Federal University of São Paulo, BP Hospital, São Paulo, SP.Study goal: To find patterns of coronary lesions more common in individuals with diagnosis of Neoplasia in the presence of ACS. If found, the possibility of performing angiographic criteria for the analysis of new coronary angiography would help in the understanding of coronary disease and provide a basis for further research in the area and for the treatment of patients. In addition, it is intended to estimate the prevalence of factors predisposing to the development of ACS in cancer patients, to evaluate the degree of complexity of the coronary anatomy and to relate these data to the development of intrahospital complications.
