Abstract

The emergence and spread of drug-resistant bacteria has become a global public health problem that has compromised the effective treatment of an increasing range of infections that affect humans and other animals, also impacting environmental health, and, therefore be addressed as a One Health problem. In this regard, the high prevalence and endemicity of bacterial species such as Escherichia coli, Klebsiella pneumoniae, Salmonella enterica and Pseudomonas aeruginosa, in healthcare-related infections (HAI), it has been supported by the well-determined combination of virulence factors (viruloma) and the presence of antibiotic, disinfectant and heavy metal resistance genes (resistoma). In this context, the genetic determinants of resistance mobilized by plasmids stand out, with emphasis on the production of broad-spectrum beta-lactamases (ESBLs), carbapenemases (KPC, NDM) and transferable resistance to colistin (MCR). As an aggravating factor, in Brazil, the dissemination of these genetic determinants in these pathogens has occurred dynamically at the human-environment-animal interface, with a rapid adaptation of high risk clones, such as E. coli ST10, ST38, ST131 , ST224, and ST648; K. pneumoniae belonging to CC258; and P. aeruginosa ST277. Thus, using a genomic approach, the present study aims to clarify aspects related to the resistoma, viruloma, mobiloma and plasmidoma of pandemic lineages of clinically important Gram-negative bacteria from different hosts and ecosystems. To this end, an integrated genomics database with easy access, named "OneBR" (One Health Brazilian Resistance), will be created, which will enable the surveillance, diagnosis and treatment of antimicrobial resistance within the One Health concept. The benefit of this study will result from the availability of genomic data (big data) on a free platform easily accessible by professionals and researchers in the area of human, animal and environmental health, and a priori will allow to elucidate genetic aspects of adaptation, resistance and virulence of these pathogens considered of critical priority by the world health organization (WHO), circulating in the human-environment-animal interface, in Brazil.
