Rapid molecular test: effectiveness in diagnosing tuberculosis in vulnerable populations

Abstract

This study aims to analyze the performance of care provided for the diagnosis of tuberculosis (TB), from the perspective of the effectiveness of the rapid molecular test in vulnerable populations, specifically, among people living with Human Immunodeficiency Virus (HIV) infection and people deprived of liberty (PDL). It is an investigation that involves three types of study: descriptive, transversal and ecological study of time series. For the study, variables will be collected from the TB-WEB information system referring to TB cases residing in 21 municipalities in the state of São Paulo that have implemented the rapid molecular test. In addition, data will be collected from the medical records of cases treated at the Hospital das Clínicas in Ribeirão Preto and who were examined for the diagnostic hypothesis of TB. Such data correspond to: identification data; data to define the study population; sociodemographics; clinics; diagnostics; treatment outcome. Structured and semi-structured interviews will also be carried out with managers to understand the context of the municipalities in which the rapid molecular test is used. Data will be analyzed using frequency distribution, univariate analysis and prevalence ratio; the agreement of the test results with the results of sputum smear microscopy and culture will also be calculated. Finally, a time series analysis will be performed to analyze the effectiveness of the test implementation in the trend of empirical treatment and treatment failure. For analysis of qualitative data, content analysis will be performed. It is believed that the contributions of this research involve the production of theoretical and operational knowledge that will allow reflections on the strengths and weaknesses of the aforementioned technology in priority scenarios for coping with the disease, as well as the capacity to produce social and health responses aimed at detection of TB cases in people living with HIV and people in situations of deprivation of liberty. Studies of this nature are pertinent, as they will address the feasibility of rapid molecular testing in the context of health practices, as well as the identification of organizational arrangements that permeated the incorporation of that technology, aiming to qualify the care provided to vulnerable populations.
