Abstract

Periodontal disease is a multifactorial disease which is influenced by factors such as the presence of periodontopathogenic microorganisms, the reaction of the immune system, smoking, genetic susceptibility, stress and the occurrence of systemic diseases. The inflamed periodontal tissue produces several cytokines like Interleukin 4 (IL-4) and Interleukin 8 (IL-8). Recent studies carried out by this group investigated IL8 and IL4 gene polymorphisms in patients with and without periodontitis. In those studies were observed individuals with a genetic predisposition to periodontitis even after adjusting for covariates such as age, gender, skin color and smoking (results under submission to publication). The purpose of this study is to test the hypothesis that the major genetic susceptibility to periodontitis due to the carriage of some IL8 and IL4 haplotypes would be linked to differences in the cytokine levels and increased rates of periodontopathogens. One hundred and sixty individuals will be selected and grouped according to their genetic susceptibility into "Susceptible" and "Non-susceptible" groups, while into each group there will be patients With and Without Periodontitis. Also, we intend to verify whether individuals carrying a specific haplotype will show a better response to periodontal treatment and whether this would be influenced by the production of cytokines and/or by the presence of periodontopathogens. The selected individuals will be submitted to new periodontal exams, and the gingival fluid will be collected before the periodontal treatment and again 45 days after it. The IL-8 and IL-4 cytokines will be quantified through immunoenzymatic assays. It will be analyzed by absolute quantification the levels and proportions of Porphyromonas gingivalis, Tannerella forsythia and Treponema denticola by Real Time PCR. This study is expected to contribute with a better understanding of the potential inter-relationship of different factors such as genetic, immunological and microbiological with periodontitis, since this disease is multifactorial.
