Abstract

Chronic Kidney Disease (CKD) patient's mortality, even after adjustment for traditional risk factors is 10 to 20 times greater than the general population, mainly by Cardiovascular Disease (CVD). The chronic inflammatory state is one of the most important nontraditional risk factors for general and cardiovascular mortality. The relationship between CKD and inflammation is established in the early stages of renal disease. Inflammation is a recognized causal factor for the development and acceleration of atherosclerotic disease. Inflammatory biomarkers, such as interleucin-6 (IL-6) and C-reactive Protein (CRP) are associated with atherosclerosis and mortality (general and cardiovascular) in CKD patients. Intravascular Ultrasound (IVUS) has evolved in recent years as a valuable adjunct to angiography. IVUS provides a unique method for studying the morphology of atherosclerosis in vivo. There are no studies evaluating the relationship between inflammation and atherosclerotic disease severity assessed by IVUS in CKD patients. Main hypothesis: As inflammation is enrolled in the development of atherosclerosis, we can predict the atherosclerotic disease severity (assessed by IVUS) using inflammatory biomarkers. Specific aims: To evaluate the relationship between coronary atherosclerotic disease (presence and severity) assessed by IVUS and the chronic inflammatory state in CKD patients under hemodialysis.
