Validation of the Portuguese version of the telephone interview for cognitive status among stroke patients

Abstract

Data show that stroke as the main public health problem in Latin America and the leading cause of deaths in Brazil. Besides the physical and motor limitations, 30% of stroke patients may develop neuropsychological changes, most of times it associated with a cognitive impairment and to progress to dementia. Cognitive impairment affects significantly the functional rehabilitation, and contributes to an increased risk of mortality by up to three times in these patients. Early diagnosis of cognitive impairment and/or dementia allows more effective therapeutic interventions. However, the instruments currently available in the cognitive assessment may present difficulties in clinical and epidemiological studies. An initial cognitive assessment performed by telephone implies to reduce costs and time. This study aims to validate the Brazilian version of the Modified Telephone Interview for Cognitive Status Assessment (TICS-M) for assessment of cognitive impairment and dementia in patients after stroke. Methods: The adaptation process will be conducted in accordance with the following phases: Phase I: translation of the original English version into Portuguese by an informed person (T1) and one without prior knowledge about the subject (T2); Step 2: Synthesis the two translations for discrepancies resolution version (T1-T2); Step 3: Retro-translation to the original version in English (two versions) based on the version T1-T2; Step 4: Review and production of the pre-final version; Step 5: The instrument must be applied to a sample of 30 subjects of general population to validate the version. Finally, it will performed the validation of the current Portuguese version of the TICS-M among a subsample of 90 consecutive cases of stroke (first episode) after 6 months of the event from a prospective cohort (Study of Morbidity and Mortality of Stroke- EMMA). Patients will be classified into three groups: no cognitive impairment, with cognitive impairment (mild and moderate) and with dementia. The TICS-M will be faced with the following instruments, which will be considered the gold standard: specific cognitive tests, Mini Mental State Examination (MEEM) and Montreal Cognitive Assessment (MoCA). To measure the reliability of the TICS-M will be used Cronbach´s alpha, with the aim of evaluating the homogeneity of the instrument ( internal consistency), and test-retest reliability will be determined by Intraclass Correlation Coefficient. Concurrent validity will be assessed by examining construct overlap with other cognitive and functional measures controlling age, education, and gender. In regard of discrimination validity, the means of test scores were compared among Without Cognitive Impairment, With Cognitive Impairment and Demential Syndrome groups by analysis of covariance (ANCOVA) controlling age and education levels with post hoc contrasts with Tukey's honestly significant difference (HSD) method. Receiver operating characteristic (ROC) curve analysis will be performed to investigate discrimination validity of the TICS-M, MEEM, MoCA and specific cognitive tests. Areas under the curve (AUC) of ROC curves will be compared among the TICS-M and these three cognitive instruments. (AU)
