Implants primary stability assessment by radiographic and tomographic examination, histology and frequency of ressonance: in vitro study

Abstract

In Implantology, one of the main factors for success of the osseointegration is the primary stability of dental implants in bone of different densities. This study aims comparatively assess the initial stability of four different types of self-tapping implants, controlling the variability factors in bone, in an in vitro model evaluating: Insertion Torque, Resonance Frequency Analysis (RFA), Push-out Test and Histomorphometric Analysis.For this, 100 implants will be divided into 5 groups, each with 20 implants: Group 1 - XiVE® PLUS implants; Group 2 - Ankylos® implants; Group 3 - Straumann Tapered Effect implants; Group 4 - Straumann Bone level implants; and Group 5: Biomet 3i NanoTite implants. Fifty implants (10 of each group) will be placed in 50 blocks (one for block) prepared and standardized from fresh pig bone (different densities will be used to simulate human bone density - high density and low density bones). The bone blocks will undergo a certification process prior to the study, in order to ensure uniformity of bone density. The remaining 50 implants (10 in each group) will be placed in 50 blocks (one for block) of polyurethane (artificial bone, high density and low density). The implant placement will be performed as recommended by the manufacturer. This study will comprise 4 experiments: Experiment 1 (polyurethane blocks - D1 low-density) and Experiment 2 (polyurethane blocks - D3 high-density): after installation of implants, Insertion Torque, Resonance Frequency Analysis (RFA) and Push-out Test will be conducted; Experiment 3 (Block of Pig Bone - Low-density) and Experiment 4 (Block of Pig Bone - High-density): after installation of implants, Insertion Torque, Resonance Frequency Analysis (RFA), radiographic bone density (measured by subtraction radiography), bone/implant contact (BIC) and bone density of the adjacent and distant areas from the implant surface will be evaluated. The results of each implant groups will be compared, evaluating if there are statistically significant differences (ANOVA or Kruskal-Wallis test, p <0.05) between them, regarding the evaluated parameters.
