Abstract

Invasive urothelial carcinoma is a highly lethal disease. Currently, the treatment is guided by the biological behavior and, in particular by the level of invasion. While the superficial tumors are amenable to local resection, the muscle-invasive are treated with cystectomy. In these cases, neoadjuvant chemotherapy is indicated, but there is a high rate of chemoresistance. The last decade has brought important advances in understanding the molecular mechanisms involved in the oncogenesis of urothelial bladder carcinoma, motivating the search for prognostic or predictive markers of therapeutic response, especially those subject to targeted therapy. Hypoxia-induced and mTOR signaling pathways are closely related and are involved in the oncogenesis of many cancers, including invasive urothelial carcinoma. However, the activation status and the interaction of these pathways at different stages of progression of urothelial carcinoma are not yet well established, being based on less comprehensive and fragmented studies. The pharmacological inhibition of both pathways, alone or in co-inhibition is possible and is approved in other organs of the urinary tract, making them promising therapeutic alternatives. This study aims to analyze the activation status of mTOR and hypoxia-induced pathways by immunohistochemistry and RT-PCR Real-time and the expression of miRNAs in urothelial carcinomas correlating the results with clinical and pathological parameters.
