Influence of cervicitis caused by Chlamydia trachomatis in the persistence of HPV infection: role of cytokine production in the infection site

Abstract

The genital HPV infections are transient, and most of them are eliminated by the immunological system of immunocompetent women. Although only a small proportion of women show persistent HPV infection, the high prevalence of the virus makes it an important public health problem, particularly in developing countries. Although the characteristics of the inflammatory process triggered by Chlamydia trachomatis infection are well known, little has been reported about that such pathogen plays as regards HPV elimination by the immune system. Hence, this study aimed at evaluating the influence of cervicitis by C. trachomatis as a co-factor for risk for persistent HPV infection and at characterizing the profile of expression of cytokines expressed on the infection site. It is an epidemiological study that will track down approximately 3.300 women aged 18 to 50 years, treated at the Primary Health Care Units in the city of Botucatu. During specular examination using the Collins bivalve speculum, which will be sterilized and free of any lubricants, vaginal content will be collected for analysis of the vaginal microbiota pattern, following the criteria by Nugent et al. (1991) and Donders et al. (2002). Next, cervical secretion will be collected by cytobrush for HPV and C. trachomatis evaluation by Polymerase Chain Reaction (PCR). Neisseria gonorrheae evaluation, also by PCR, will be performed for exclusion of patients showing cervicitis by that agent. HPV-positive women will be divided into 2 study groups, in the presence or not of a history of C. trachomatis infection, and they will be followed for identify the prevalence of viral persistent infection for a 24-month period. These data will be correlated with the cytokine expression profile expressed on the site of infection, which will be evaluated by ELISA. (AU)
