Objective sleep changes pre and post total parathyroidectomy in patients with secondary hyperparathyroidism on hemodialysis

Abstract

Patients with end-stage renal disease (ESRD) are surviving mainly due to renal replacement therapy and renal transplantation, and usually have several comorbidities. Among those comorbidities, bone and mineral metabolism have great impact on morbidity and mortality in these patients. Sleep disorders are common in patients with chronic kidney disease, especially on dialysis. Sleep complains are largely documented, and are present in 50 to 80% of these patients. However, sleep disorders in patients with symptomatic secondary hyperparathyroidism are poorly reported. One study showed that sleep complains in this situation are up to 97%. In that study insomnia was correlated to the severity of pruritus, weakness, and levels of parathyroid hormone (PTH). Patients with more severe hyperparathyroidism had more sleep complains. Parathyroidectomy (PTX) can improve the subjective symptoms of hyperparathyroidism. One study in ESRD patients on hemodialysis, with insomnia, who underwent PTX observed an improvement of PTH levels and an increase of total sleep time, accessed by questionnaires. So far, there is no study demonstrating objectively what happens with sleep after a PTX. The objective of this study is to access the clinical improvement that happens in patients with ESRD and secondary hyperparathyroidism after PTX, correlating to polysomnography data. (AU)
