Randomized clinical study of photodynamic therapy in the periodontal treatment of patients with AIDS and HAART failure

Abstract

Periodontal disease is directly related to the presence of biofilm and associated with the host immune response. HIV patients had a change in bacteria that is composed of gram-negative anaerobic bacteria associated with the host immune deficiency. Photodynamic therapy (PDT) has been shown to be a promising alternative and viable in reduction of periodontopathogenic bacteria. The aim of this study is to evaluate effectiveness of PDT in periodontal disease of patients with Aids and Highly Activity Antiretroviral Therapy - HAART failure. Twelve HIV patients with genotype showing resistance to various combinations of HAART divided into two groups (test and control), and 12 patients with no evidence of genotyping resistance to various combinations of antiretroviral therapy are divided into two groups. We will evaluate the clinical periodontal parameters as full-mouth plaque score (FMPS), full-mouth bleeding score (FMBS), local plaque index (PI), bleeding on probing (BoP) on experimental sites, probing pocket depth (PPD), gingival recession (REC), clinical attachment level (CAL) at baseline and after 3 and 6 months after treatment. In addition, we will perform a collection of subgingival biofilm at baseline and 3 and 6 months after treatment. As evidenced recently in the scientific literature PDT is a viable therapeutic option, low cost and randomized controlled clinical trials are needed to assess the impacts of different types of patients with degrees of immunosuppression, such as AIDS patients on HAART failure.
