Abstract

Heart transplantation is the most effective treatment for children with advanced heart failure. Mechanical support with Ventricular Assist Devices (VADs) allows the majority of children to be bridged to heart transplantation, improving the efficiency of organ use in children and adolescents and reducing the waiting list mortality. The most common form of mechanical support available for high risk pediatric patients is the extra corporeal membrane oxygenation (ECMO) which is mainly recommended for short-term use. The aim of this research is the development of a VAD and an ECMO system suitable for short and long term cardiac or respiratory support of pediatric patients. This proposal encompasses the development of a pediatric VAD (pVAD) with optimal flow dynamics applied to patients under 25 kg body weight, the development and test of new surface treatment and functionalization to reduce thrombo-embolic and infeccious complications, a high-efficiency extra-corporeal membrane oxygenation system for respiratory and cardiac support and the pre-clinical evaluation of the pVAD. The life-saving technologies to be developed in this proposal are expected to improve the outcomes of mechanical circulatory support with VAD and ECMO in children and lower the risks and costs associated with it.
