Identification, oxacillinase characterization and molecular typing of Acinetobacter calcoaceticus-A. baumannii complex clinical isolates from the State of São Paulo, Brazil

Abstract

Healthcare associated infections lead to increase of morbity and mortality, and can reflect in the costs of patients' management. Acinetobacter species, particularly those included in the Acinetobacter calcoaceticus-A. baumannii (ACB) Complex, are associated with several kinds of nosocomial infections, and represent an important treat to infection control. They possess a large number of resistance mechanisms against virtually all available antimicrobial agents, including the carbapenems, which had been the longstanding last-line agents for treating Acinetobacter infections. The main mechanism associated with carbapenem resistance is the beta-lactamase production of OXA enzymes. A global increase in carbapenem-resistant Acinetobacter has been reported. Polymyxin has presented high activity against Acinetobacter isolates although some resistant-isolates have had reported, including in Brazil, which lead to need of monitoring this resistance phenotype. Elucidation of clonal relationship of circulating Acinetobacter strains is desirable to monitor the spread and implement measures for controlling their dissemination. Phylogenetic analyzes, in addition, can provide information regarding the evolution and features that contribute to persistence and spread of successful strains. The aim of this study is to provide the characterization of circulating strains isolated from clinical specimens of hospitalized patients across the State of São Paulo, identifying the species, determining the antimicrobial susceptibility and defining the clonal and phylogenetic relatedness among them.
