Abstract

Low birth weight (LBW) is a heterogenous group of newborn infants, that are small because they were born prematurely or suffered intrauterine growth restriction. There is great concern about these babies since the incidence is growing and mortality is high. Breastfeeding is the gold standard in the term or preterm infant nutrition. However, the direct effect of gestational age (GA) or fetal growth on the mother's milk composition is not known yet. Objective: To evaluate the composition of colostrum and mature milk from mothers of LBW preterm infants born appropriate or low birth weight for GA, and the growth of these infants during the first month of life. To investigate the effect of intrauterine growth restriction on the mother's milk composition and the infant growth. Methods: A prospective observational and cross-sectional study, involving mother's of LBW preterm infants, delivered alive at University Hospital- Botucatu School of Medicne, UNESP. The study will be done after approval by the Ethics Committee. Inclusion criteria: mothers without diabetes, not using drugs, singleton birth, 30-36 weeks' gestation, 1000-2499g birth weight, neonates without congenital abnormalities. Exclusion criteria: colostrum sample not obtained, mothers in use of medication. A sample size of 60 LBW preterm infants in each group was estimated to show significantly differences according to prematurity and fetal growth. According to birth weight, the LBW preterm infants will be divided into 2 groups: appropriate or low birth weigh for GA, and then compared to a control group (term appropriate for GA). The milk samples will be obtained in the first 72 hours (colostrum) and in the 4th week of life (mature milk). Variables to be evaluated: maternal sociodemographic characteristics, prenatal and perinatal data, neonatal outcome and the growth during the first month of life. Main outcome: I- Components of colostrum and mature milk, including energy, macronutrients, IgA, inflammatory mediators and oxidants/antioxidants markers. II- Anthropometric data and nutritional evaluation at birth and in the 4th week of life. Statistical analysis: Comparisons among the groups will be done using the Chi-square test for categorical variables and ANOVA for continuous. Simple and multiple linear regression will be used to evaluate the effect of group on infant growth. Significance level at 5%. (AU)
