Treatment of residual pockets with Photodynamic therapy in patients with aggressive periodontitis history. Clinical, microbiological, immunoinfalmatory and patient-centered analysis

Abstract

The aim of this split-mouth and randomized clinical trial is to evaluate through clinical, microbiological and immunoinflamatory analysis the effect of photodynamic therapy (PDT) in the treatment of residual periodontal pockets in patients with aggressive periodontitis history. Thus, 20 patients will be selected presenting at least 2 posterior teeth with residual periodontal pockets with probing depth (PD) e 5 mm and bleeding on probing (BOP). In order to determine the effect of PDT, the teeth will be randomly distributed into 2 groups: PDT Group (1 tooth): photosensitizer Application (10mg / ml methylene blue) + sweep with the tip and laser activation (diode laser 660 nm , 60mw) for 1 minute per site, TC Group (1 tooth): surgical access to ultrasonic debridement associated with manual scraping; SRP+RAR Group (1 tooth): Scaling and root planning + photosensitizer Application (10mg / ml methylene blue) + sweep with the tip and laser activation (diode laser 660 nm , 60mw) for 1 minute per site; SRP Group (1 tooth): Scaling and root planning. The following clinical parameters will be evaluated prior to therapy (baseline) and after 3 and 6 months: Plaque Index (PI), BOP, position of the gingival margin (PGM), PD and relative clinical attachment level (RCAL). The microbiological evaluation will be proceeded by PCR-real time (A. actinomycetencomitans, P. gingivalis, C. gingivalis, E. corrodens, T. forsythia, P. micros e P. intermédia) at baseline, after 3 and 6 months. The immunoinflamatory analysis will be made by Luminex/MAGpix platform (IL-1², IL-4, IL-6, IL-8, IL-10, IL-17, TNF-a and INF-g at baseline, 3 and 6 months. Subsequently, data will be tested for normality (Kolmogorov-Smirnov test) and the statistical methods (parametric or non-parametric) will be indicated. A significance level of 5% will be adopted for all analysis. (AU)
