The management of the third period of the labor and their repercussions in the postpartum

Abstract

In the year of 2000, the World Organization of Health (OMS/WHO) it published estimates in relation to the world maternal mortality. According to the data, there are 400 alive deaths approximately maternas/100.000 childbirths in the world, in Brazil in 1996, they happened 161 alive maternal deaths/100.000 childbirths, totaling 8700 deaths/year. It is considered that at least, 25% to 30% of those deaths are caused due to postpartum hemorrhage, and the sanguine loss is directly associated with the management of the third period, the speed and the uterine contraction. Nowadays, two forms of clinical approach of the management of the third period exist. Physiologic and active. The active management understands the following interventions: profilatic administration of oxitocyn endovenous or intramuscular, section of the precocious umbilical cord; controlled traction of the umbilical cord and massage in uterine bottom. In spite of the active management to be a suitable strategy for the World Organization of Health for the prevention of the postpartum hemorrhage and reduction of the maternal mortality, and its effectiveness already it was proven for several studies in different countries, we can notice, that in practice, the services of health that attend the childbirth have not been adhering to this management type, maintaining as conduct still today, the expectant handling. This way, we looked for through this study, to observe how it is being accomplished the handling of the third period of normal childbirths and their repercussions in the postpartum. (AU)
