Asses the readiness of preterm infants for the start breastfeeding: correlation between the score and performance in relactation

Abstract

One great difficulty for the health caregivers in the assistance of the premature is to define the ideal time to transition from gastric to oral feeding, raising the need of a comprehensive evaluation and sucking behavior of the baby. An instrument for assessement of the readiness of preterm infant to the start of breast feeding has been validated for its content, appearance (FUJINAGA et al., 2008a), reliability (FUJINAGA et al., 2007) and clinical validation (FUJINAGA, et al., 2008b). The instrument is divided into categories (corrected age, state of organization behavior, posture oral, oral reflexes and non-nutritive sucking) and each has items with scores 0-2, with a maximum score of 36 points. As a continuation of the validation process, the present study has as an objective to verify the discriminant validity of the instrument to assess the readiness of preterm infant for the transition from gastric to oral feeding, correlating the score obtained after application of the instrument with the ingestion of milk-mediated relactation. The study will be correlational type. The sample will be composed of 54 dyads of mothers and premature babies, which meet the inclusion criteria, from a neonatal intermediate care unit. The evaluation instrument will be applied before the premature has started been fed orally. Then, the performance of the baby will be verified by putting he/she to suck his/her mother's breast, mediated by relactation which will enables to objectively quantify the volume of milk ingested. Kappa coefficient will be used to test inter-rater reliability between two observers (researcher and author of the instrument), and linear correlation coefficient of Pearson will be used to check the discriminant validity.
