Abstract

Obesity is a multifactorial disease in which is verified a pathologic increase of adipose tissue associated with a low intensity chronic inflammatory state. Obesity is considered a worldwide epidemic disorder and represents one of the major challenges in public health due to its increase in prevalence and incidence, besides many complications and comorbidities associated. The systemic arterial hypertension (SAH) is one of the main comorbidities associated with obesity, 50% more prevalent in obese people. The left ventricular hypertrophy (LVH) is an extremely important risk factor for cardiovascular morbidity and mortality and it is the center of many studies that correlate the cardiac hypertrophy with hypertension and with obesity. The LVH is often divided into two groups: concentric or eccentric. The concentric hypertrophy is observed when left ventricle is submitted to an overload pressure due to a persistent increase in afterload. The eccentric hypertrophy is traditionally associated with a volumetric overload and it is, therefore, related to obesity since it establishes a chronic estate of volumetric overload in order to maintain the blood flowing through the excessive adipose tissue deposit. It is believed that the combination of hypertension and obesity provides a superior additive effect to the mechanism of hypertrophy than each by their own. However it is unclear which one is the main responsible in sever obesity. The purpose of this study is to correlate the patterns of cardiac hypertrophy in sever obesity according to the classification of obesity and severity of the systemic arterial hypertension.
