Identification of patients subgroups with non-specific chronic low back pain which respond better with method Pilates

Abstract

One of the current research priorities in low back pain is the treatment based in subgroups of patients. The Pilates method has been recommended for the treatment of chronic low back pain, but there is no evidence on its effectiveness in different subgroups of low back pain patients. The objective of this study is to identify subgroups of patients with chronic non-specific low back pain, according to the classification system of the clinical practice guideline, that get better benefits with the treatment based on the Pilates method. We will evaluate 296 patients with chronic non-specific low back pain, aged between 18 and 80 years of both genders, which will be included in a randomized controlled trial that will investigate the effectiveness of the Pilates method with different weekly frequencies in the treatment of chronic non-specific low back pain. The outcome measures will be pain intensity, general and specific disability, global impression of recovery and kinesiophobia. For the classification of the subgroups (mobility impairment, mobility coordination impairment, pain associated with lower limbs and affective cognitive trends), the clinical findings will be identified after clinical history and physical examination, as well as pain severity will be classified in acute, subacute or chronic. The analysis will consider the clinical outcomes assessed in the randomized controlled trial in subgroups classified according to the system adopted. Data will be analyzed with accuracy statistical tests, receiver-operator curves and regression analyzes. We believe that the subgroups of mobility impairment, mobility coordination impairment and pain associated with the lower limbs will benefit more from the Pilates method than the cognitive affective subgroup.
