Association between deficiency or insufficiency of vitamin D and periodontitis

Abstract

The deficiency or insufficiency of vitamin D has been increasing globally in the past few years, may resulting in alterations in bone metabolism and in the immune and inflammatory response, possibly impacting periodontal disease. The aim of this study is to investigate serum concentration of 25(OH)D (vitamin D) in individuals with periodontal disease compared to healthy subjects. It will be recruited to participate in this study 72 individuals 18-69 years, both genders, presenting at least 10 teeth. The participants will be divided into two groups: DP+ (n= 36) - diagnosis of periodontitis, determined by the presence of at least 2 interproximal sites in different teeth with attachment loss (AL) > 4mm or 2 interproximal sites in different teeth with probing depth (PD) > 5mm; DP- (n= 36) - diagnosis of absence of periodontitis, with PD < 3mm at all sites and < 10% of bleeding sites. For that, participants will answer a healthy questionnaire and will be periodontally examined according to PD, AL, full-mouth gingival bleeding index (GBI) and plaque index (PI). All participants will be asked to perform a panoramic X-ray and collection of blood sample for complete blood count, fast glucose and serum 25(OH)D analysis. Plasma concentration of 25(OH)D will be classified as deficient (< 20 ng/mL), insufficient (21-29 ng/mL) or sufficient (> 30 ng/mL). Panoramic X-ray images will be digitized and analyzed according to mandibular cortex index (CI), anatomic morphology of the mandible posterior to mental foramen and radiographic interproximal bone height. Data will be compared between groups by Student t test and Mann Whitney. Plasma concentrations of 25(OH)D e the proportion of participants with deficiency or insufficiency of vitamin D will be analyzed by Wilcoxon, Mann Whitney and c2. The association of vitamin D deficiency and moderate to severe periodontitis will be investigated by multiple logistic regression analysis. The correlation of 25(OH)D concentrations and PD, AL, GBI and PI will be investigated by Pearson and Spearman correlation and c2 tests, with a 5% significance level. (AU)
