Wear analysis after treatment of occlusal lesions of molar-incisor hypomineralization in enamel: randomized and controlled clinical trial

Abstract

Molar-incisor hypomineralization (MIH) is a qualitative enamel defect that affects one or more permanent first molars and may also be accompanied by alterations in the incisors. This condition affects the patient's oral health, aesthetics, and quality of life. Clinically, hypomineralized enamel presents with demarcated opacities that may fracture or result in increased wear depending on the porosity of the surface. A project supported by FAPESP evaluated the effectiveness of the type of treatment for occlusal surface defects in permanent first molars with MIH in children aged 6 to 10 years who had at least one permanent first molar with MIH enamel defects on the occlusal surface. The children were treated with diode laser irradiation and occlusal sealing with high-viscosity glass ionomer cement (GIC), and initial molds and polyester resin models were obtained at 12 and 30 months after the treatments. Based on the data obtained, greater changes were observed in teeth with MIH, although this was not the project's objective. Since it was possible to store the images in STL format for future analysis, this project aims to evaluate them in terms of occlusal and cusp wear of teeth with MIH, comparing them to teeth without MIH. The teeth are divided into three groups based on diagnosis: a control group (CG) with molars without MIH, a group of molars with MIH treated with GI sealing (GC, EQUIA Forte Fil), and a group of molars with MIH treated with laser. Scans were obtained from the models. The 3D meshes obtained by the scanner will be analyzed by overlapping 3D data sets using an interactive best-fit method in Medit software. Based on the overlap data, occlusal wear rates of molars from all groups will be calculated. The data will be tabulated, analyzed, and the best statistical method will be chosen for use.
