Resumo

Um dos maiores avanços no tratamento das doenças reumatológicas foi o surgimento dos bloqueadores do fator de necrose tumoral (anti-TNF), utilizados no Brasil há somente cerca de cinco anos. Isso ficou bem comprovado para a artrite reumatoide, artrite idiopática juvenil e artropatias soronegativas sugerindo que existem alvos terapêuticos comuns nessas doenças. Atualmente, três agentes anti-TNF estão aprovados para o uso em nosso país: infliximabe, um anticorpo quimérico consistindo de uma porção variável Fab murina covalentemente ligada à fração constante de IgG1 humana; etanercepte, um dímero geneticamente construído do receptor solúvel do tipo II do TNF, no qual o domínio extracelular p75 foi fundido com a região Fc da IgG1 humana; e o adalimumabe, um anticorpo monoclonal totalmente humanizado contra a molécula de TNF. No entanto, essas medicações estão associadas a um risco aumentado de infecções comuns e oportunistas e neoplasias. Além disso, existem inúmeros aspectos envolvendo órgãos alvos tais como os efeitos do anti-TNF sobre os ossos, dentes, coração, pulmão, aparelho reprodutor e rins que ainda não foram estudados. Nesse sentido, o presente projeto propõe abordar os aspectos peculiares desses imunobiológicos em doenças reumatológicas. (AU)
