Resumo

Breast cancer (BC) is the leading cause of cancer-related deaths among women in Brazil, with an age-standardized mortality rate of 12 deaths per 100,000. Around 40% of BC diagnoses in the country occur at advanced stages (TNM III-IV), disproportionately affecting brown and black women with lower education levels. Recent data indicated an increasing trend in BC mortality in São Paulo, following more than two decades of declines. In response, the ConeCta-SP (Cancer Control in the State of São Paulo) project was launched in 2022 to establish an organized and population-based screening programs for cervical and breast cancer in São Paulo. The initiative addresses key barriers to BC screening through specific strategies, such as optimizing workflow organization, training healthcare teams, actively reaching the target population, increasing public awareness, and fostering partnerships with companies, universities, and schools. Additionally, a monitoring platform is being implemented to track screened women, facilitating follow-up and reducing delays in diagnosis and treatment. As part of ConeCta-SP, this proposal aims to continuously evaluate the performance of the implemented BC screening program using key indicators, including: mammography coverage; participation rate; proportion of exams performed outside the target age population and recommended screening intervals; compliance with follow-up assessment; rates of true-positive, false-positive, and inconclusive mammography results; and time from positive screening to diagnosis. This proposal will contribute to the assessment of evidence-based strategies aimed at enhancing the long-term sustainability and scalability of the organized BC screening program within São Paulo's public health system.
