Resumo

The objectives are: to analyze the reporting method for medication errors, including adverse effects; to analyze the opinions of the heads of the administrative, medical and pharmaceutical staffs, nurses and the nursing personnel concerning error reporting; to identify and analyze the errors presented in reports in terms of type, frequency, time of occurrence, medication, category of professional involved and the described situation; to identify and analyze medication errors occurring in a hospitalization clinic which were detected through the methods of observation, chart revision and report revision; to propose and implement a computerized or manual form for error reporting with anonymity. The data will be collected in five hospitals. The Ethics Committee had approved the development of this project. (AU)
