Experimental trial of the eficacy of the use of acetobacter's cellulose dressing to avoid restenosis on the scar tissue of surgical treatments of anterior glottic comissure webs in minipigs

Abstract

Glottic stenosis is a fibrosis taking to scar tissue and narrowing of the larynx, can attain the whole organ or segmental extension. The stenosis can come from traumas as automobile and sporting accidents, from orotracheal intubation and may happen, with an undesirable assiduity, after the surgical manipulation of the larynx, after resection of tumors, papillomas, Reinke’s edemas and stenosis. There are indications that about 10% of the patients submitted to bilateral surgery in anterior glottal comissure develop stenosis in the resection site. The treatment of the stenosis is one of the main problems of the field. To treat the anterior glottal stenosis we have endoscopic or external procedures. Both demand some manipulation, with ressection or debridment of the adhered tissue, with cold instruments to or with laser, following by placement or not of substances that avoid or reduce the scar formation as esteroids or mitomicina C and insertion or not of molds and keels between the damaged areas. Once the cases of anterior stenosis of the glottis can happen after the manipulation of the justacomissural mucous membrane of the vocal folds, being a problem of difficult resolution, demanding multiple interventions, with placement of molds and its removal, it is necessary to find alternatives that avoid or help to reduce the chances of adherences after these procedures. Objective: To evaluate the effectiveness in avoiding the re-stenosis, with the use of the temporary dressing of acetobacter’s cellulose in the ressection area after the treatment of anterior glottic stenosis, in minipigs. Material and method: The animals to be used will be minipigs BR1, of 6 months of age and weight of 30 to 35 kgs. For the control group we will have 10 animals and for the study group we will have 10 animals. Initially the animals will be submitted to scarification of the anterior one third of both vocal folds to produce a local stenosis. After 30 days the minipigs will be revalued and if they present adherence among the vocal folds they will be allocated in the study groups or control group, and have their adherences undone by surgical ressection and, in agreement with the allocation group, placement or not of cellulose dressing on both scar areas. The animals will be followed for 60 days when they will be appraised again and sacrificed for histological study of their vocal folds. (AU)
