Abstract

Oral mucosa is a major site of complications following hematopoetic progenitor cell transplantation (HPCT). Recently, oral mucositis was described as occurring in 100% of short time transplantation patients submitted to high dose chemotherapy followed by HPCT, and is described as responsible to high morbidity and development of infections. Mucositis can be evidenced around day +10 after beginning the conditioning regimen, presenting major intensity between day +7 and day +11 and can remain present during 2 to 3 weeks. Methotrexate (MTX) is an anti-folate drug used to prevent graft versus host disease (GVHD) and play a major role in mucositis and mielosuppression. Metilenotetrahidrofolatoreductase (MTHFR) is an enzyme that regulates folate metabolism and is the main target of MTX. Enzymatic specific activities can be resumed according to genotype as the wild genotype (CC) that remains in activity of 100% of MTHFR, genotype CT (heterozygous) reduced in 35% of the activity and CC (homozygous) with a reduction of 70% of MTHFR activity. These polymorphisms can play a major role in MTX toxicity in oral mucosa of patients that use this drug as GVHD prophylaxis. Furthermore it is important to establish a relation between MTX toxicity and MTHFR polymorphisms in oral mucositis following HPCT. (AU)
