Abstract

Healthcare associated infection (HAI) is of global concern due to mortality, morbidity, and its associated costs. Surgical site infection (SSI) is currently responsible for 20% of the global HAI. Considering the global number of surgeries (234 million) and its ancillary complications volume, an intervention on SSI's occurrence is crucial to reducing HAI globally. Both HAI and inadequate antibiotic indication might affect bacterial resistance, which rises every year. It's estimated that over 40% of antimicrobials are indicated as surgical prophylactic, and that more than half is unwittingly prescribed. Considering the antibiotic volume used with this purpose, and its probable association with antibiotic resistance and SSI, coupled with the limited number of national publications on the subject, it is relevant to study the prevalence of antibiotic prophylaxis use and institutional strategies to improve its compliance. This study may provide information on the adherence to recommendations, frequent inadequacies, and best results initiatives, supporting strategies to improve practices. This study aims to verify surgical antibiotic prophylaxis compliance, and identify facilitators and barriers to compliance. We intent to carry out a cross-sectional study surrounding São Paulo city hospitals. This will include adult patient records of both genders, undergoing clean surgery, with surgical antibiotic prophylaxis indication, to remain in hospital during the antibiotic prophylaxis indication. We will exclude outpatient surgery, emergency surgery, and that carried out on patients during antibiotic use. We will describe hospitals, healthcare infection control commissions, and institutional surgical antibiotic prophylaxis practices, and programs. The sample size, based on the inclusion criteria, considering public and private hospitals, with an error of 3%, and estimated loss of 20% is 1167 procedures. (AU)
