Lung function and the prevalence of asthma among children and adolescents with sick cell anemia

Abstract

Sick cell anemia is a chronic hemolytic anemia with frequent and severe pulmonary complications. Lung function abnormalities are common among children with sick cell anemia, mainly the obstructive ones. Children with sick cell anemia have high prevalence of asthma and the relation between these two diseases is not yet well understood. The diagnosis of asthma in patients with sick cell anemia is associated with higher morbidity and mortality due to pulmonary complications. Surprisingly, very limited information is available regarding lung function and asthma diagnosis in Brazilian children with sick cell anemia and these are the objectives of this study. The diagnosis of asthma will be established after a cross-sectional evaluation of respiratory symptoms, allergic sensitization, exhaled nitric oxide and lung function in a group of 100 patients (6 to 18 years-old). Lung function will be measured by spirometry and by impulse oscillometry. Impulse oscillometry is a quite recent technique that measures pulmonary resistance and reactance in a simple and non invasive way. This technique provides information that is additional to spirometry and has demonstrated to be more sensitive in the detection of obstructive abnormalities and in the evaluation of peripheral airways. No information has already been published on impulse oscillometry in children with sick cell anemia. Our hypothesis is that impulse oscillometry may contribute to the evaluation of pulmonary complications in patients with sick cell anemia and in the understanding of its relation with asthma. (AU)
