Abstract

Aortic Stenosis is one of the most common valvular heart disease in adulthood. It corresponds to over 4.5% of the elderly, resulting in a high number of older people who needs cardiac surgery. One of the highest challenges of clinical practice is how to recognize the cases which will most benefit from aortic valve replacement. The challenge seems to be even higher in those who present small valve area, associated to low pressure gradient and left ventricular dysfunction. Those patients are potentially severe and lots of them do not have a good response after surgery intervention. Further more, many cases are still under diagnosed and are interpreted as moderate aortic stenosis, and it can result in misconduct. The aim of this study is to identify the value of new diagnostic methods like tridimensional echocardiography and cardiovascular resonance imaging with T1 mapping in the diagnosis of low flow low gradient aortic stenosis. Secondary objectives consist in the evaluation of late prognosis of these patients, through evaluating cardiac biomarkers, left ventricular reverse remodeling, left ventricular function and relative percentage of fibrosis in the late post-operative period comparing to the pre-operative period.
