Abstract

Patients undergoing cardiothoracic surgery with cardiopulmonary bypass (CPB) show inflammatory reactions triggered by this procedure and by ischemia - reperfusion, which lead to pulmonary dysfunction and injury of vascular endothelium postoperatively. These processes related to cardiac surgery may alter the production and consumption of nitric oxide (NO) in lung and vascular endothelium, mainly the participation of NO in the inflammatory process. In this study we will evaluate the exhaled fraction of NO (FeNO) in patients undergoing cardiac surgery in the preoperative and postoperative periods (4 hours, 12 hours, 24 hours and 48 hours after CPB). In this way we intend to verify: the influence of cardiopulmonary bypass on FeNO; the influence of nitrates over the FeNO in the preoperative and postoperative periods; verify that FeNO levels are useful as a marker of respiratory complications after cardiac operations with cardiopulmonary bypass; verify if the FeNO from patients with heart valve disease differs from patients with coronary disease and to verify the possibility of obtaining FeNO in patients undergoing invasive mechanical ventilation adapting the device NIOX MINO® in the expiratory limb of the ventilator. From the results we intend to elucidate the role of NO as a biomarker of lung injury and possible changes FeNO resulting from surgical procedures and the use of medications mentioned. (AU)
