Evaluation of the inflammatory profile in children with adenotonsillar hypertrophy and sleep disordered breathing

Abstract

Respiratory disorders during sleep have been recognized among adults and children since long. They range from snoring to states of obstructive sleep apnea (OSA) with numerous clinical implications. Obesity is a known risk factor for OSA in adults. The prevalence of OSA in overweight (obese) children reaches up to 36%, hitting 60% when habitual snoring is present. Although inflammatory consequences are known in adults, in children these consequences are still not well defined.Aim: to evaluate clinical reports of obstructive breathing disorders during sleep in obese and non-obese children and to evaluate the inflammatory perfile in children with adenotonsillar hypertrophy and sleep disordered breathing, comparing the obese and non obese ones. Material and Methods: children aged between 3 and 12 years, diagnosed adenotonsillar hypertrophy and sleep disordered breathing will be included. A questionnaire (OSA18) will be applied in order to investigate snoring, observed pauses in breathing and restless sleep. All children will be submitted to otorhinolaryngological exam for diagnosis of hypertrophy of the tonsils, as well as a sleep study (polygraph)in accordance to standard criteria of the American Academy of Sleep Medicine (AASM). The inflammatory evaluation will be carried out by the measurement of fasting TNF-alpha, cytokines 6 and 10. Results will be desribed in median and standard desviation for obese and non-obese children, with and without OSA. The correlation of the inflammatory findings to OSA and obesity will be analyzed.(AU)
