Abstract

Diabetes mellitus is considered an important factor that may negatively interfere on the periimplantar bone healing. Nevertheless, the effect of glycemic control on bone healing around implants in subjects with diabetes is unclear. The aim of this clinical, randomized, prospective trial is evaluate the influence of glycemic control of type 2 diabetics on dental implants stabilization. Indeed, this study aims to evaluate the pattern of gene expression of molecules involved in the bone metabolism, according to the glycemic status. Forty five subjects having the potential to benefit from dental implant therapy will be recruited: Non-diabetics (n=15); better-controlled type 2 diabetics - glycated hemoglobin levels d 8% (n=15), worse-controlled type 2 diabetics - glycated hemoglobin levels > 8% (n=15). Before implant placement, bone tissue biopsies will be harvested from the sites designed to receive one-stage dental implants. Gene expression of TNF-a, TGF-², RANKL, OPG, Runx2, ALP, BSP, COL-I and OCN - essential bone markers - will be evaluated by quantitative real-time polymerase chain reaction (PCR). After implant placement, implant stability will be obtained by resonance frequency measurements at baseline and following 3, 6 and 12 months. The results will be compared using ANOVA or Kruskal Wallis, after normality test, with the level of significance set at 5%.
