Optimizing head CT indications in motorcyclists victims of mild head trauma

Abstract

The number of traffic accidents involving motorcyclists increased significantly in the last few years. In some cases, mild trauma victims may have serious intracranial injuries. If not identified in time, these lesions may be cause of high morbidity and even death. This assures the liberal CT scan indication in these patients. In order to decrease the number of negative exams, there is an increasing search for the best selection criteria. However, the proposed algorithms are based on data from international studies, which might not reflect the Brazilian reality. Objective: Identify a group of motorcyclists involved in traffic accidents carrying very low risk of intracranial injuries, in whom the head CT scan would not be necessary. Method: We will carry out a prospective study, collecting data of motorcyclists involved in traffic accidents, including those ejected from the vehicle, in use of closed helmet, who have Glasgow Coma Scale equals 15 at admission, were conscious, oriented and have no factors that alter the results of physical. All patients included in the study will be submitted to a head CT for diagnosis of intracranial lesions. The frequency and the severity of all lesions and their treatment will be assessed. Expected results: We intend to identify a group of patients who do not require head CT, what would reduce radiation exposure and costs, as well as optimize the indication of CT in emergency departments.
