Association between serum BUN concentrations evolution in the first 48 hours after hospital admission with return of spontaneous circulation and with in-hospital mortality

Abstract

In-hospital cardiorespiratory arrest (IHCR) are serious events that occur annually in 1 to 5 out of every 1000 hospitalizations and, although they have a high mortality rate, most cases do not occur suddenly, so up to 70% of PCRIH could have been avoided. Thus, biomarkers and scales to stratify prognosis are interesting approaches. For example, studies have already shown the association between IL-6 and C-reactive protein with the post-arrest outcomes. Thus, the present study aims to evaluate urea, as a biomarker, which results from protein catabolism and is a marker of neurohormonal activation. In addition, urea is excreted by the kidney and is increased in patients with chronic kidney disease or acute kidney injury. Therefore, our hypothesis is that changes in serum urea values in the first 48 hours of hospitalization are a prognostic indicator for patients who have IHCP. This study aims to analyze 200 medical records of patients who were victims of IHCP treated by the Rapid Response Team of Botucatu Medical School Hospital, and to evaluate demographic, clinical, and laboratory data, in addition to the association of urea variation in the first 48 hours of hospitalization with the outcomes return to spontaneous circulation and in-hospital death.
