Comparison between Swenson and Soave's surgical techniques for transanal colorectal pull-through using an experimental model in rabbits: which approach has the best trans and postoperative evolution?

Abstract

Introduction: The first surgical technique for treating Hirschsprung's disease (HD) was described in 1948 by Swenson and Bill. Since then, some variations and innovations have been proposed. Currently, there is a tendency to give preference to transanal techniques, with specific surgical details that seem to imply the postoperative evolution of patients. However, there is no consensus as to whether the transanal surgical procedure is the most appropriate for treating HD. Objective: to compare aspects of peri and postoperative evolution after performing the Swenson and Soave techniques for transanal pull-thought in rabbits. Materials and Methods: Experimental model in 25 rabbits using two different transanal pull-through surgical procedures. Primary, the animals will be randomized, divided into two experimental groups (Group 1 - 10 animals using transanal Swenson technique and Group 2 - 10 animals, transanal Soave technique) and one simulated control group (Sham Group, with five animals ). We will monitor intraoperative data. After the experimental and simulated procedures, we will monitor the animals for 30 days to evaluate clinical aspects and possible complications. On the 31st day after the procedures, the animals will be euthanized. We will analyze the results of the different groups in a comparative way. Expected results: we hope to identify which surgical technique of transanal pull-through is more appropriate, with shorter surgical time, lower complication rate, and better postoperative evolution. The results found can suggest future trials in human beings, with changes and adjustments in the conduct in the face of HD treatment.
