Suicidal ideation in patients with Crohn's disease.

Abstract

Suicide is an important social and public health problem in contemporary societies. Every year, around 800,000 people around the world die by suicide: one in every 100 deaths. Every year more people die from suicide than HIV, malaria or breast cancer. The two main forms of inflammatory bowel diseases (IBD) are Crohn's disease and ulcerative colitis (UC), immune-mediated conditions characterized by chronic inflammation, with periods of remission and exacerbation, whose etiopathogenesis is still not fully elucidated. In the last decade, there has been an increase in the number of studies published worldwide, relating psychological aspects to inflammatory bowel diseases (IBD). The results showed that psychiatric disorder is higher in individuals with Crohn's disease (CD) than the general population, including a higher suicidal ideation (SI). The suffering experienced by patients with CD, such as chronicity, the constant use of medication, hemorrhages and the fear of the possibility of new hospitalizations indicate that they facilitate the onset of depression and increase the suicide rate. Another factor strongly related to the risk of suicide in patients with CD and UC is pain, predominantly abdominal, frequent in more than 80% of patients with IBD. Despite the data published in the world literature suggesting a strong association between IBD and psychological aspects, there are few studies involving SI in countries where IBD is considered emerging, such as Brazil. In the IBD outpatient clinic of the Hospital das Clínicas of the Faculty of Medicine of the University of São Paulo (HCFMUSP), the association of psychological disorders and IBD has often been observed, which motivated our growing interest in the topic. The aim of the present study is to estimate the prevalence of SI in patients with CD treated at HCFMUSP and to assess whether there is a relationship between SI and gender, age group, marital status, smoking, religion, previous surgeries and disease duration.
