Abstract

The overall incidence of hospitalization for Community-Acquired Pneumonia (CAP) is high and associated with significant morbidity and mortality. Thrombotic complications, including venous, arterial, and possibly microvascular, occur in patients hospitalized for various CAP etiologies. In addition, adverse outcomes in patients with pneumonia may be mediated by inflammatory and thrombotic processes, which lead to respiratory, cardiac, and other organ dysfunction.This prospective cohort study, which will be carried out in the ward of the Hospital das Clínicas de Bauru, Bauru, São Paulo, intends to evaluate the association between serum levels of D-dimer on admission and prognosis (in-hospital mortality, dead in 28 days, need for Intensive Care Unit and orotracheal intubation) of patients over 18 years of age hospitalized for less than 72 hours with a diagnosis of pneumonia.We hope to recruit at least 200 patients to demonstrate that patients with D-Dimer > 500 ng/mL are 10 times more likely to have in-hospital mortality than patients with D-Dimer considered negative.This study hopes not only to evaluate a prognostic marker, but also to contribute scientifically to knowledge about coagulopathy in patients with pneumonia.
