Abstract

With the aging of the population, degenerative aortic valve stenosis has become increasingly common, resulting in an increase in valve prosthetic implants. However, some patients continued to have an increasing transvalvular gradient in the immediate postoperative period. This elevation may be a consequence of a Prosthesis-patient mismatch (PPM), which occurs when the valve's effective reach area is small for the patient's body surface. Other possibilities are hypervolemia and prosthetic thrombus. The maintenance of a high gradient after the perioperative period is usually related to PPD.The present study aims to analyze the impact of transprosthetic pressure gradient values in the recent postoperative period on late clinical and hemodynamic results, in addition to evaluating the factors associated with the greater risk of high gradient in the postoperative period of aortic valve replacement. This is a retrospective observational study in which all patients undergoing valve prosthesis implantation in the aortic position will be selected in a quaternary hospital specialized in cardiology located in the south of São Paulo, which have clinical, surgical and echocardiographic data available in the system. of electronic medical records in the period from 2010 to 2020. Your data will be found and purchased in a spreadsheet in the Excel program (Microsoft) for subsequent analysis.
