Abstract

Idiopathic esophageal achalasia (IEA) is an inflammatory disease that causes dysphagia and consequent malnutrition. Conservative treatment is recommended when in its advanced form, although there is still controversy. The mucosectomy technique has been used for Chagasic achalasia, showing improvement in swallowing and when complications present, they have low morbidity. In this context, this study proposes to use mucosectomy as a surgical treatment in patients with an advanced degree of idiopathic disease after presenting a recurrence of conventional surgical treatment. Even so, it is known that mucosectomy is not a treatment free from complications, with dehiscence of the esophagogastric anastomosis at the cervical level being the one with the greatest morbidity. That said, this study aims to compare the early and medium-term results of manual suturing techniques in one plane and mechanics, used in cervical esophageal anastomosis, only in patients of idiopathic origin. Four patients diagnosed with relapsed advanced idiopathic achalasia, who underwent previous surgery and who present clinical conditions to undergo esophageal mucosectomy, will be evaluated.
