Abstract

Introduction: In the last decade, major institutions have come together to understand the living conditions that affect the population's health, known as "Social Determinants of Health (SDH)." These include the conditions in which people are born, grow, live, work, and age, which are influenced by a set of economic, social, and political forces and systems. Few studies have examined the interaction of SDH with diagnosis and prognosis in psychiatry, especially in Obsessive-Compulsive Disorder (OCD) and related disorders (OCRD). Objective: To analyze SDH in families of probands with OCD/OCRD, using a dimensional approach to identify the factors that best describe the interrelationships among them, and to classify subgroups of subjects who share an SDH profile. We will investigate the association of these factors/subgroups with the clinical profile of OCD/OCRD and family type across different regions of Brazil. Additionally, we will assess whether the SDH factor scores of family members can predict the clinical profile of the probands. Finally, we will create social vulnerability indices for the study subjects and examine whether these are associated with OCD/OCRD symptoms. Method: A total of 900 patients with OCD/OCRD and 600 first-degree relatives will be recruited, divided across three regions of Brazil: Northeast, Southeast, and South. All participants will be assessed for the presence of psychiatric diagnoses and SDH, and subjects with OCD/OCRD will be evaluated for symptom severity. We will use factor analysis and hierarchical agglomerative clustering (HAC) and, subsequently, regression models to observe the relationship between factor scores/groups and the clinical profile of OCD/OCRD and family type, and whether parental SDH predicts the clinical profile of the probands.
