CORRELATION OF THE INFLAMMATORY PROFILE AND BONE MARKERS WITH THE VOLUME OF PERIAPICAL LESION IN TEETH WITH PRIMARY ENDODONTIC INFECTION

Abstract

The pulp tissue is exposed to various aggressors that can lead to its inflammation.In the absence of intervention, necrosis of this tissue occurs, characterizingprimary endodontic infection (PEI), which may lead to periapical inflammationwith the development of apical periodontitis (AP). In turn, this periapicalinflammatory process induces the release of cytokines involved in the regulationof osteoclastic activity. The interaction between RANKL, RANK, and OPG isessential for osteoclastogenesis, while osteoblasts and osteocytes play vital rolesin bone remodeling. Thus, this study aims to analyze the expression of thecytokine TNF-¿, as well as the bone markers sclerostin, osteocalcin, OPG,RANKL, and FGF23 in the periapical fluid of patients with periapical lesions,correlating these data with clinical symptoms and the lesion volume obtained bycomputed tomography. The study will include 30 teeth from adult patients withPEI and AP, who will undergo clinical and tomographic exams for accuratediagnosis, analyzing the symptoms prior to treatment and the volumetricassessment of periapical bone destruction. Coronary access will be performedwith spherical diamond tips, and the canals will be prepared using a singlereciprocating file with irrigation using 2.5% sodium hypochlorite. After foramenenlargement with Kerr file #30, periapical fluid (SF) samples will be collected toassess cytokines and bone markers, which will be analyzed later by multiplexassay. The data obtained will be evaluated using descriptive and inferentialstatistical tests.
