Abstract

The incidence and prevalence of hepatocellular carcinoma (HCC) is growing worldwide, leading to high mortality rates. The disease occurs mainly in cirrhotic patients, and the cure can be reached only when it is diagnosed early or when the tumor is submitted to adjuvant therapies to reduce the tumor size, being the transarterial chemoembolization (TACE) the most used approach to achieve this aim. Among the TACE limitations, it can not be used for patients with portal vein thrombosis (PVT), which is a frequent complication in cirrhotic patients who have HCC. Furthermore, there is a big controversy on how to compare results obtained from different locoregional therapies as TACE and transarterial radioembolization (TARE), a treatment strategy that has been successfully applied for patients with HCC because it allows reducing the tumor size even in cases of PVT. The aim of this study is to compare the efficacy of TACE and TARE on the reduction of the target HCC tumor applying a software that was specifically developed by this group of researchers, allowing to measure the tumor before and after the treatment with great accuracy on computed tomography exams. Forty subjects with intermediate HCC submitted to TACE or TARE procedures will be evaluated. The study will be prospective, randomized and only patients with PVT will be direct to TARE without previous randomization. The locoregional response, the adverse events and the survival rates will be measured and compared between TARE and TACE, leading to suggestions about the individualization of these patients' treatment.
