USING PATIENT REPORTED OUTCOME MEASUREMENTS FOR MANAGING PATIENTS WITH HEARING LOSS - A RANDOMIZED CONTROLLED TRIAL

Abstract

Background: There is a growing consensus about using patient-reported outcomes (PROMs) as a tool for the continuous improvement of medical services, value-based payment and innovation in patient-centered clinical flows. However, the impact that the collection of PROMs may have in terms of direct clinical outcomes for patients and health system efficiency has not yet been fully proven. Recent studies in the USA have shown some benefit in their use for monitoring the symptoms of cancer patients, even improving patients' long-term survival. However, the context in which these studies were performed is not suitable for inferring on their cultural transposition to other patient groups and to the Brazilian clinical practice. Aim: This project aims to evaluate the impact of a clinical flow based on patient reported outcomes reported in patients with hearing loss in the Department of Otorhinolaryngology of Hospital das Clínicas da Faculdade de Medicina da USP (ORL-HCFMUSP) in terms of its effectiveness (improving patient follow-up) and its efficiency (reduction of resource consumption). Methods: The project will include two phases: 1. Scientific consensus on the questionnaires (PROMs) to be used to measure quality of life in patients with deafness; 2. a randomized controlled study with patients from the Otorhinolaryngology Outpatient Clinic of ORL-HCFMUSP, monitored through an electronic monitoring platform of outcomes (PROMs selected in 1.) vs. usual follow-up with protocols used in our department. Scientific innovation and societal contribution: this project constitutes the first randomized controlled study to evaluate the impact of PROMs on ENT; considering the high volume of patients treated in ORL-HCFMUSP, the monitoring of PROMs, if it demonstrates impact on the improvement of patient follow-up, could be a clinical innovation with a high impact for the Health System.
