Abstract

This project aims to promote the early detection of subclinical rheumatic carditis by echocardiography using the innovative criteria proposed by the World Heart Federation in 2012, joining echocardiographic, clinical, phonocardiography and immunological findings.The prevalence of this neglected disease will be estimated, filling a gap in our literature. This project has socioeconomic importance, since early detection can prevent the development of complications such as rheumatic valvular heart disease, the leading cause of heart failure and heart valve surgery in the youthful age, reducing the costs of the National Health System and increasing survival.Patients will be selected in the area of socioeconomic vulnerability in a community in São Paulo. 4200 patients will be evaluated by echocardiography and cardiac auscultation. After that, we will select two groups of patients (with intermediate and definitive criteria for rheumatic carditis) and a third group (normal, control) according to clinical and echocardiographic evaluation. These subgroups will be evaluated for immunogenetic profile for rheumatic disease.Therefore, this project is expected to validate the criteria of WHF in our population, to estimate the prevalence of subclinical rheumatic carditis, associating with different diagnostic methods.
