Abstract

S. aureus is an important agent of bloodstream infection, surgical site and pneumonia among liver transplant patients. Data from the Unit of Liver Transplant of Hospital das Clinicas of University of São Paulo showed that S. aureus was the most frequent bacterium isolated from 104 liver transplant patients between 2002 to 2005 being responsible for 17% of nosocomial infections. The molecular characterization of S. aureus resistant to oxacilin (MRSA) identified in the liver transplant unit is important to define strategies that can be useful to control and prevent the spread of this agent. The present study will be performing in three phases. The diagnose of problem, in this step, the prevalence of patients waiting for liver transplant colonized by S. aureus resistant to oxacilin and the incidence of nosocomial infection due to this agent will be determine. All strains of MRSA identified before and after the liver transplant will be evaluate phenotipically and genotipically (genes of resistance and virulence) by different techniques including microarray. The intervention will be based on the results of molecular typing, if the transmission of MRSA was due to cross-infection, contact isolation will be reemphasized, if the source of infection was the own nose colonization of patient, the decolonization will be tried. During the third phase the rates of infection due to MRSA pre and post liver transplant will be comparing. (AU)
