Middle ear reflectance and otoacoustic emissions in different tympanometric patterns

Abstract

The search for effective methods in the early diagnosis audiological is a great concern for the professionals. The goal of screening is to identify possible changes of the hearing as early as possible. The otoacoustic emission tests identify outer hair cells dysfunctions that affect the cochlea. However, the measurement is influenced by the middle ear status. The reflectance of the middle ear is a diagnostic procedure able to detect discrete dysfunctions of the middle ear, not detected by timpanometry, improving the predictive value in identifying changes in middle ear that affect the otoacoustic emissions. Objective: To identify discrete changes of the middle ear not detected by timpanometry, that could be associated with fails in otoacoustic emission tests. Method: The sample of this study will be composed of 75 individuals aged between 15 and 30 years for female and male gender, comprising two groups: GROUP 1 - 45 subjects with middle ear disorders arranged in three subgroups according to tympanometric results: type B, C and Ad (15 subjects in each subgroup), acoustic reflexes absent and hearing thresholds up to 35 dB. GROUP 2 - 30 subjects without middle ear diseases detected by tympanometry (curve type A), acoustic reflexes present from 500 to 2000 Hz and normal hearing thresholds (0 to 20dB). Procedures: Once ranked the Groups 1 or 2, through the inclusion criteria, the participants will be submitted to: transient and distortion product otoacoustic emission, reflectance energy wideband from 200 Hz to 6000 in the intensity of 0.80 dB SPL. Results: The variables will be subjected to statistical analysis for comparison between groups, procedures and ears evaluated. It is expected that the results confirm the initial hypothesis of the study, that the reflectance is more sensitive in identifying changes in the middle ear with respect to tympanometry probe of 226 and 1000 Hz.
