Abstract

The optimal dose of corticosteroid to be used in intraarticular injection is not well established. The objective of this study is to compare the effectiveness and tolerance in the medium term between small and large dose of triamcinolone hexacetonide (TH) used in joint intraarticular injection (IAI) of small, medium and large size joints of patients with rheumatoid arthritis (RA). Materials/Methods: A controlled, randomized, prospective, double-blind study will be realized in patients with RA, where they will be evaluated joints representing small, medium and large (MCF-metacarpophalangeal, wrist and knee respectively). They will be recruited 180 patients from the outpatient clinics of Rheumatology, UNIFESP. Patients will be randomized (Clapboard randomization) in two groups of 30 patients each (large and small dose of HT). Only one joint is injected by patient (IIA blindly) each time. Evaluation will be conducted by a blinded observer at five times: baseline (TO), one week (Ti), four (T4), eight (T8) and twelve (T12) weeks, with the following instruments: visual analogic scale (VAS) for pain, VAS for swollen, Goniometry, Modified Stanford health assessment questionnaire (HAQ), clinical disease activity index (CDAI), percentage of improvement, Likert Scale, dose of anti-inflammatory and corticosteroids used for joint pain studied, morning stiffness, VAS for pain at the time of HA, VAS worse after IAI, local side effects, clinical evaluation of the patient.
