Tooth transposition, root dilaceration and taurodontism: a radiographic study in a population of individuals with cleft lip and palate

Abstract

Craniofacial development and dental development are both an interaction between genetic factors and environmental factors. However, genetic factors appear to influence these developments more than environmental factors. In particular, interference of the development in each of these tissues may lead to cleft lip and palate as well as other dental anomalies. Furthermore, cleft lip and palate have complex etiology and morbidity and represent the most common birth defects. Additionally, the syndromic cleft, which accountants for 30% of all cases, relates to phenotypes that characterize a syndrome; whereas, non-syndromic cleft, which accountants for 70% of all cases, is determined by a single phenotype unrelated to other alterations.The most common dental anomalies in individuals with cleft lip and palate are tooth agenesis and supernumerary teeth. However, recent international studies have highlighted other anomalies such as root dilacerations, tooth transpositions and taurodontism that are of significant clinical importance in the rehabilitation process. The prevalence of these anomalies is increased in individuals with cleft lip and palate when compared to the general population. There are few reports about taurodontism, root dilacerations and tooth transposition in the literature. Thus it is proposed in this study to investigate these anomalies and determine their prevalence in a group of patients with non-syndromic cleft lip and palate.This study will be a retrospective study that will analyze the panoramic x-rays (panoramic) of 1000 individuals aged 18 years or older. These x-rays will be divided into four equal groups which will include the following: unilateral cleft lip and palate, non-syndromic; unilateral cleft lip only, non-syndromic; cleft palate only, non-syndromic; and a control group without cleft. Radiographs will be selected according to a specific inclusion criteria, digitized and then analyzed using specific software.One examiner will assess each dental anomaly following pre-determined criteria, and will be previously trained using a pilot study. Next, data will be analyzed using descriptive statistics. Finally, statistical tests will be used to compare different types of cleft lip and palate and to compare females with males. (AU)
