Relation between immunogenetics and environmental factors in a Brazilian epidemiological study of Pemphigus vulgaris and Pemphigus foliaceus

Abstract

Current reports confirm clinical-epidemiological modifications related to increased incidence of pemphigus vulgaris (PV) in endemic regions for pemphigus foliaceus (PF), also affecting young individuals, configuring endemic PV. Genetic factors, especially related to HLA molecules of class II -DR and -DQ, allied to environmental factors, such as virus, heavy metals and insects, in the case of PF, and pesticides in PV are implicated in the susceptibility to the disease. However, the literature does not provide evidence or significant epidemiological studies, configuring only sporadic reports. The present project has as main objective to relate immunegenetics and environmental factors to the ethiopathogenesis of PV and PF. This thematic project is composed by 6 subprojects, that contemplate the specific objectives: (1) analysis of immunegenetics factors: prevalence of antibodies against desmoglein (Dsg) 1 and Dsg3; and of HLA of class II -DR and -DQ molecules in patients' samples with PV and PF, family and neighborhood; and study of the cells Treg in patients with PV and PF, compared to control group; (2) analysis of environmental factors: correlation between titles of antibodies and of the antigens of herpes simplex virus (HSV -1 and 2), and virus of dengue; prevalence of seric antibodies against virus transported by hemathophagous insects (arbovirus), responsible by vesicular stomatitis in bovine - vesiculovirus Indiana and New Jersey -, and virus Piry; and prevalence of the exposition to pesticides and heavy metals in patients' samples with PV and PF, family and neighborhood, and their relationship with Aquifer Guarani and the rivers Moji-Guaçu and Pardo. The subprojects reflect on classic analyses of prevalence of autoantibodies against desmogleins and of determination of the HLA Class II molecules, whose results will have essential implication for the analysis of the results of the other subprojects. In relation to the prevalence of antibodies and viruses antigens commonly seen in human diseases, and the exposition to pesticides and heavy metals, we are looking for the corroboration of sporadic reports. Finally, the project concerning vesiculovirus is plenty innovative when we will search for virus transported by insects related to the pemphigus, however described to be present only in cattle, and still no related to the man. The results of each subproject will be analyzed individually, and all the results will be analyzed together, using specific software for multivariate analysis. (AU)
